Transitioning from Pediatrics to Adulthood
The transition from Pediatrics to Adult care happens at 18 years of age or when a child becomes emancipated. This transition should be started by picking a new provider and requesting that your medical records are sent there. This can be done by filling out a records request form in person and giving it to a receptionist.
In Utah County there are many Family Medicine, Internal Medicine, and OB/GYN providers.
