What is Transitioning in Care?
Alpine Pediatrics is committed to helping patients make a smooth and informed transition from pediatric
to adult health care. Alpine Pediatrics understands that this transition comes with many questions, and
our pediatricians are happy to help answer these questions. This process involves working with youth,
beginning at ages 12 to 14, and their family to prepare for a change from a "pediatric" model of care,
where parents make most decisions, to an "adult" model of care, where youth take full responsibility for
decision-making. This means that our Providers may spend some time during the visit with the teen
without the parent present. This will allow the teen to open up, help the provider set health priorities,
and support them in becoming more independent in their healthcare decisions.
At age 18, youth legally become adults. Alpine Pediatrics wishes to respect that many of our young
adult patients choose to continue to involve their families in healthcare decisions. Only with the young
adult's consent will Alpine Pediatrics be able to discuss any personal health information with any
family members; this includes if the patient is living away from home at college, is serving an LDS
mission, or is otherwise away from home. Even if a young adult is on an insurance plan the parent
carries, they must choose to disclose their health information to anyone other than themselves in
writing. If the youth has a conditions that prevents them form making healthcare decisions, we
encourage parents and caregivers to consider options for supported decision-making.
When will my child transition from pediatric to adult care?
Alpine Pediatrics will collaborate with youth and families regarding the age for transferring to an adult provider, and it is recommended that this transfer occurs before age 22. Once a patient reaches the age of 22, Alpine Pediatrics can no longer see them as patients. Alpine Pediatrics will assist with the transfer process, including helping identify an adult healthcare provider, sending medical records when the patient has selected an adult healthcare provider, and communicating with the adult provider about the patient's unique needs and health concerns.There are scenarios where the patient may be required to transition to an adult provider before age 22. These scenarios include if a patient gets married or becomes a parent before age 22, Alpine Pediatrics can no longer accept the patient. Once an individual gets married or becomes a parent, they encounter unique health scenarios that no longer fall inside the specialty of a pediatrician and are better suited for an adult medical provider.
Family Medicine
Family Medicine is an excellent option for most patients. A Family Medicine doctor is the traditional "GP" and is trained in and specializes in treating patients through all phases of life.
